A Targeted Case Manager (TCM) supports children, families, and individuals by helping them access the services and resources they need to thrive. In this role, you work directly with clients to understand their challenges, create personalized care plans, and connect them with helpful supports such as therapy, school services, medical providers, and community programs. You’ll advocate for their needs, coordinate with other professionals, and make sure each person receives consistent, meaningful help across every part of their life. This position is perfect for someone who is organized, compassionate, and motivated to make a real difference in the community.

Candidates must have a Bachelor’s Degree in a behavioral health or human services field, at least one year of full-time, post-degree experience working with the SMI/SED/SUD population, have a valid driver’s license, and be willing to travel up to 25 miles.

CSAs provide direct support to children and families, helping them build skills, access resources, and navigate challenges in their daily lives. We’re looking for compassionate, reliable individuals who are passionate about making a positive impact and enjoy working hands-on in the community. Candidates must be at least 18 years of age, have a High School Diploma, at least one year of full-time experience working with individuals who receive services for mental health or co-occurring disorders, have a valid driver’s license, and be willing to travel up to 25 miles.
